Transcript Requests

To request a transcript, please call the CHS School Counseling Office at 814-473-5318 or email Mrs. Destarta Carberry, CHS Building Secretary at [email protected]. 

Information needed to process the transcript request:  

• Year of graduation or current grade level
• Maiden name or name at graduation
• Official or unofficial (note: official transcripts must be mailed directly to the college/university or place of employment)
• Email, fax, and/or address of where to send the transcript
• Your contact information in the event we have a question processing your transcript.
